What type of information will ACSF need to evaluate the case?
ACSF may request additional information after reviewing your application. The additional information may include such things as the police report (if your case involves a motor vehicle accident), witnesses statements, a copy of the emergency room record, a copy of the discharge summary for any in patient hospitalizations, a recent narrative report from your treating doctor, an expert's report if your case involves medical malpractice, a copy of the Complaint, if one has been filed by your lawyer, etc.
